Fukasaku,M.D. have designed a new instrument to use hydro stream
power to split the nucleus.
The lens fibers run meridionally around the equator from the posterior to the anterior lens surface. It is important for the surgeon to mentally visualize these meridional fibers. These are the stress lines. Hydro chopping will create the vector forces along this stress line and the subsequent splitting of the nucleus will occur along this stress line.
This cannula is 23G and vertically or perpendicularly thin to introduce into the nucleus easily. This cannula has two small 0.2mm holes at both sides and attached in a disposal standard syringe with BSS. By pushing the inner cylinder, a small amount of BSS is blown off on both sides. This spurting hydro jet create vector forces along the meridional lens stress line. We call this cannula as Hydro Chopper.
This hydro chopper is buried in to the nucleus through the 1.4mm corneal incision. If the nucleus is soft, it is easy to introduce the hydro chopper into the central nucleus. However, for the hard nucleus more than grade 3, the surgeon needs to hold the lens with snapper hook at the counter part. The hydro chopper can be easily introduced into the harder nucleus with holding at the counter side.
By pushing the inner cylinder of the syringe, a small amount of BSS is blown off as a hydro jet through the 0.2mm holes on both sides and acts as a vector forces to split the nucleus into two pieces.
The nucleus is then rotated for quartering. Hydro chopper is introduced
into the half nucleus. With small amount of BSS hydro jet, the half
nucleus is split into quarters.
Phacoemulsification and aspiration of the quartered fragments then proceeds.
Phacoemulsification can be performed with ease within the central safe
zone. Because the nucleus is already split into quarters, micro incision
bimanual phacoemulsification procedure can be much easier, safer and
faster over the previous MICS technique. This hydro chopping technique
makes MICS surgery much easier, safer and faster.
